Description
Patient diagnosed with HTN, hx stroke, Rheumatoid Arthritis
Equipment: shower chair, bedside commode, wheel chair
Hours: 8pm-8am Saturday, Sunday, Wednesdays
Saturdays 2pm-8pm Sundays 8am-8pm
(6 HOUR SHIFTS ALSO AVAILABLE)
All applicants must have:
-Driver's License/Passport/Permanent Resident Card
-Social Security Card (Original Card)
-Current Physical (Stamped or Dr. License number, within a year)
-Current PPD (within a year) if positive - chest x-ray
-MMR Titers
-License/Certification
-CPR Training Card
-Malpractice Insurance (NSO.com) LPN's, RN's, PT & SP)
-3 Reference Letters (2 Business, 1 Personal)
-If 4 years or more on the job: 1 Business & 1 Personal
